I thought it warranted mentioning that there are other instances that can lead to increased glucose levels in the blood. Medications such as epinephrine, birth control pills, corticosteriods, and diuretics can cause elevated levels...in addition to renal failure, pancreatitis, and over eating. One reason for the pancreas failing to produce insulin is due to the destruction of the insulin producing cells by the immune system. But there are instances where the pancreas produces insulin yet the body doesn't recognize it and the tissues don't respond to it. Also, in a healthy person, fasting for 3 days results in low glucose levels.
