well, its a long long shot, but i am sending ni an application to the extreme makeover home edition, but i ran into something that i dont know about on the form.
on the very last page are two lines

what the heck do these pertain to, and what do i put in them?

By refers to the peson filling out the form.
It's refers to your ownership of the home i.e. Owner, tennant, daughter of owner...

I hate to disagree with NC, but I think the "By" and "Its" refers to the officer of Lock and Key Productions who excepts and signs for the production company. At least that's how I read it on page 11.

Production company on the left, owner on the right of the page.

Its: Associate Producer

We may be be talking about two differant things here. Hate contracts with so much reading the fine print.
