Your grandmother dies and your grandfather marries again, probably a much younger woman. Then he dies and you marry the widow. Let's call the speaker "John" to make things simpler.
John married a widow who has an adult daughter. So John has a stepdaughter.
John's father married John's stepdaughter. So John's stepdaughter is now also his stepmother.
Since his stepdaughter is also his stepmother, his wife (stepdaughter's Mom) is his stepgrandmother.
As husband to his "grandmother," he is his own grandpa.
All the other details of the song are funny but not necessary for him to be his own grandpa.

She is also the mother and the sister of my father (though she is no longer a sister).
How does that work out? My Aunt's father died early on, my Grandmother remarried who fathered my father. Then my grandmother passed on and my grandfather married his step daughter. Who had joined a convent earlier and had left when or because she planned to marry.
So my aunt is also my grandmother. She is also my father's sister (formally a sister to all) and now is only a sister to my father and my uncle and is also their mom. Strangely my Grandfather is also my uncle, which might make him a grand uncle, or my father's mother my grand aunt. Not to be confused with my Grand uncle's brother who is my Great Uncle (though nobody told me exactly what it was he did to become so great).
And that is no song folks, that's a real life situation.
