At first, Washington was properly spoken to as "Your Excellency," I think. I have read that he changed the address to Mr. President. "Your Highness" was also suggested, but Washington apparently didn't like that one.

I also understand Washington was called Mr. President. A term of humility, "Mr." identifies the chief executive as one of the people rather than something loftier.
