Can you name the 3 instances in MLB history of back to back to back to back HR's and the 12 players involved?

I can give you a third of it: In 1963, Woodie Held, Pedro Ramos, Tito Francona, and Larry Brown for the Indians hit back to back to back to back homers off one pitcher, Paul Foytack of the Angels. The quartet combined that year for only 35 home runs between them. And this third of the answer came from The Baseball Challenge Quiz Book by David Nemec.
