Frozen cookie dough should last a couple of months in the freezer. If you're making a cookie that requires rolling and then cutting the dough, you should wrap that in plastic wrap and then put in freezer bags, making sure to get all (or at least most of) the air out. This will keep it from getting freezer burn and taking on odors from other items in the freezer. For dough that does not requiring rolling, you can store it either in freezer bags or in containers (I'd prefer freezer bags only because it allows for less air space.)

As for the taste/freshness - they should taste just like fresh made, as long as they did not become freezer burned...

By my own experience:
- Freezer bag
- No longer than 3 days
- No, the longer you take to eat them, the worst they will taste
