Forgot to mention that various battles involved different amounts of time - Gettysburg, with the highest body count, lasted three days while (hope I remember correctly) at Cold Harbor the Union lost 10,000 troops in less than a half hour. The key is that they were all very bloody and the longer the fighting went on, the higher the body count.
