pineal cysts are actually fairly common, and are rarely significant. They hardly ever cause symptoms, and are generally left alone (the dimensions you gave are likely wrong, in that I'm about certain you mean mm not cm - 20 cm would be around 8 inches -- unheard of!!). I copied the following from a website: According to Merritt's Textbook of Neurology, benign cysts of the pineal gland are occasionally found incidentally on brain imaging studies such as MRIs. According to one study, approximately 4% of MRIs of the brain had a pineal cyst. A pineal cyst is a normal variant of the pineal gland in which a cystic structure is surrounded by normal pineal tissue. These cysts can be up to 2cm in diameter and rarely cause symptoms or compression of surrounding tissue. If you were to develop symptoms from the pineal cyst, they might include feelings of nausea, vomiting, headache, unsteady walking, double or blurry vision, memory difficulty, and lethargy. Although rare, some indications that you might need surgery are the development of symptoms, growth of the cyst, or the development of hydrocephalus. (Hydrocephalus is excess fluid around the brain, which can be caused by an obstruction of the cerebrospinal fluid flow.) The reason that the doctor may need to do surgery in these cases is that cysts usually do not increase in size or cause obstruction of cerebrospinal fluid, thus the surgeon may want to biopsy the cyst to verify it is not a rare tumor.
